摘要 目的: 1、构建与完善冠心病社区健康管理HOPE模式的具体内容和核心管理流程, 为社区开展冠心病慢病管理工作提供技术支持和理论参考。 2、以社区诊断欧亿·体育（中国）有限公司为基础,评估现阶段小河湖墅地段社区卫生服务中心实 施冠心病社区健康管理HOPE模式的可行性因素及存在的困难,并针对性这些 问题提出政策性建议与意见,为政府、卫生系统相关机构或部门制定政策、做 出决策、为社区卫生服务中心扩展工作思路提供参考。 方法: 1、采用个人访谈和专题小组讨论的形式,专访拱墅区卫生系统慢病管理相 关负责人及工作人员各12人,共24人,讨论冠心病社区健康管理HOPE模式 及该模式核心管理流程的主要内容。 2、采用多阶段系统随机抽样方法,随机抽取小河湖墅街道社区居民健康体 检欧亿·体育（中国）有限公司560份。运用“国人缺血性心血管发病危险的评估方法和简易评估工 具”,对社区居民冠心病10年发病风险进行评估。 结果: 1、通过本研究,进一步界定与完善了冠心病社区健康管理HOPE模式的主 要内容,即健康(Health)、组织(Organization)、促进(Promotion)、 评价(Evaluation)四个落脚点的具体所指。实现了冠心病社区综合管理流程、 健康挡案(HER)管理流程、健康教育与健康促进流程、家庭病床管理流程, 四大核心管理流程的构建与说明。 2、冠心病10年发病风险评估结果显示:513名调査对象中,十年发病绝 对风险大于其对应年龄段平均值的占51. 9%。相对危险度在10倍以上的有8 人,占1.6%; 3倍以上的有68人,占13.2%。社区诊断结果表明:各级政府机 关、行政部门及医疗卫生机构需加大对冠心病防治工作的重视力度。 3、通过小河湖墅地段社区卫生服务中心慢病管理现状与健康管理HOPE模 式的对照研究,得知该社区中心在实施冠心病社区健康管理的优势所在及尚存 杭州师范大学硕士学位报告 摘要 在的困难。针对这些问题,特提出对应的政策建议与意见。健康理念方面 (Health):将冠心病尚纳入社区慢病管理范畴、制定冠心病社区综合防治指 南;组织管理方面(Organization):加大政府投入力度、合理配置人力资源 配置;健康促进方面(Promotion):完善建档监管机制、促进健康教育与健 康保险有效对接、提高卫生信息化水平、健全远程心电协作机制;健康管理评 估方面(Evaluation):重视过程评估,完善评估体系。最后,通过选定试点, 以点带面,促进冠心病健康管理HOPE模式的实施与推广。 结论: 冠心病社区健康管理HOPE模式的建立为社区中心实施冠心病的综合管理 提供了一个全局性的战略规划和工作参考。诸多可行性因素对社区开展冠心病 健康管理工作提供了良好的基础,但也存在诸多困难,希望相关部门、机构予 以重视,实现冠心病的社区规范化管理和有效实施。 关键词:健康管理;HOPE模式;管理流程;可行性分析;政策建议 杭州师范大学硕士学位报告 英文摘要 Abstract Objective: 1、 Establish and perfect the specific contents and core management processes of the HOPE model of coronary heart disease health management in community, and provide technical support and theory of reference for the community to carry out coronary heart disease health management. 2、Basic on the diagnostic information of community, assess the feasibilities and difficulties in the implementation of HOPE model of coronary heart disease health management. Make practical policy recommendations and suggestions on these issues as references for government's formulating policy and making decisions and for the community to extend the work's ideas Methods: 1、Interviewed with 24 persons who are the officials and staff related to chronic diseases management from Gongshu health systems through the form of individual interviews and focus group discussions. Discuss the construction, optimization and related considerations about process of coronary heart disease health management in community and perfect the main contents and core management processes of the HOPE model of coronary heart disease health management in community 2、Extract 560 physical examination information of the residents from stream-Lake Villa community randomly,and ssess the risk of coronary heart disease among community residents in the next 10 years by the tool of risk assessment of Ischemic cardiovascular disease for Chinese people. Results: 1、Further define and perfect t the key elements of the HOPE model of coronary heart disease health management in community, namely,the specific connotations of Health, Organization, Promotion, Evaluation. 2、Among the 513 survey residents, there is one person whose absolute risk of coronary heart disease in the next ten years is more than 52.6% and 9 persons more IV 杭州师范大学硕士学位报告 英文摘要 than 21.7%,and there are 51.9 percent of residents whose absolute risk level is higher than the peers'. There are 1.6 percent of objects,namely,8 people whose relative risk level is more than 10 times than others', and 13.2 percent of objects,namely,68 people whose relative risk level is more than 3 times than others,.In General, there is a high level of major risk factors for coronary heart disease, and risk of incidence of cardiovascular disease in the next 10 years is a little higher. 3、By the research on the situation of chronic diseases in River-Lake Villa community and the HOPE model of health management, assess the feasibilities and difficulties in the implementation of HOPE model of coronary heart disease health management, and made some suggestions and comments in response to these problems. In the respect of health ideas: Make coronary heart disease is included in the management of chronic diseases in community, develop comprehensive guideline for the prevention of coronary heart disease in community. In the respect of organization: increase the financial input, configure human resources more rationally In the respect of promotion: perfect the supervision system for HER, promote the effective combination between health education and health insurance, raise the level of health information, perfect the collaboration mechanism of telecardiology. In the respect of evaluation: emphasis on process evaluation, improve the evaluation system. Finally, through pilot, promote the implementation and popularization of the HOPE model of coronary heart disease health management.
