表格2：参展资格审定表
根据国务院关于《医疗器械监督管理条理》和有关规定，所有参加展出并进行交易的展品必须具有中国国家食品药品监督管理局颁发的医疗器械产品注册证号。请详细填写此表，以便确保您的展品能够在大会期间顺利展出。

展会期间请携带医疗器械产品注册证或复印件，届时中国国家食品药品监督管理局将在现场进行检查。由于参展商未能及时正确申报展品注册号，而导致的一切后果，大会主办方恕不负责。

在展会现场我们将依据参展资格审定表的信息欧亿·体育（中国）有限公司，制作出方便需方观众查询使用的数据库文件，更好的为参展商和参观观众服务。

请使用OFFICE办公软件中的EXCEL软件制表、存盘，按照表格要求，逐项填写“展团名称、序号、单位名称（中英文）、展位号、参展展品（中英文）、规格型号、注册号等，以电子文件形式发送至主办方相关负责人。

According to the ‘ Medical Equipment Production Supervising Regulation’ promulgated by the Chinese Central Government, all medical exhibits must be registered in State Food and Drug Administration (SFDA) before being displayed and sold in Chinese medical shows.
All exhibitors participating in China International Medical Equipment Show (CMEF) should take the Registration Certificate of your medical equipment exhibits, and the officials from SFDA will do inspection around during the show opening days. Failure to submit registration information of your exhibits may result in the Authorities from SFDA ordering the closure and removal from the Exhibition hall of the exhibits or giving penalty to the exhibitor in the event, in that case the Organizer of CMEF will not be held liable for any loss thereby suffered by any exhibitor.
The Organizer will set up the Exhibitor Database in reference to this Exhibitor Registration Form in order to make exhibit information searching much easier. Please fill out the form completely and clearly in both Chinese and English, and submit the form back to the Organizer via E-mail.

表格3：新产品展示申请表
如有来不及办理注册证或正在办理注册证的新产品，请详细填写此表。大会主办方审查无误后，将发放《新产品展示证》。在展出期间，请将《新产品展示证》贴于产品醒目位置，以便现场监督检查。新产品只允许进行展示，禁止交易。如发现新产品进行交易者，视同无证处理，取消参展资格，并按国家有关规定处罚。
